Ghost Cousins is the familyship between Napstablook, Mettaton and Mad Mew Mew from the Undertale and Deltarune fandoms.

Canon

Undertale

According to Mettaton's diary, the three cousins once all lived together in the Blook Acres. However, his third diary describes that their cousin as decided to leave the farm by becoming a training dummy, causing him and Blooky to be lonely together. Napstablook then asked Mettaton if he wanted to become corporeal as well, with Mettaton noting how resigned their voice was. He told Napstablook that he should know that he would never leave his cousin behind. However, he also says that he'll never find the body he's looking for anyway. However, further diaries detail how Mettaton met Alphys, whom he initially underestimated. But Alphys showed him a design for a body for him, which he loved. He wants the body as it makes him feel more like "himself" and thinks he can never become a star when he's just a ghost. Thus, Mettaton apologizes to Blooky, telling them that his dreams can't wait for anyone.

While Mettaton became the biggest celebrity in the Underground, Napstablook spend most of their time alone and feeling like garbage. Meanwhile, it is revealed that the Dummy cousin has another cousin who had decided to become a dummy, dialogue clarifies that it's the training dummy in the ruins. It is not known whether the Ruins Dummy is related to Mettaton and Napstablook, they never mention them. Since the Mad Dummy gets mad at the child regardless of what the child did or didn't to the Ruins Dummy, it is likely that the Mad Dummy was lying about another ghost being inside the Ruins Dummy for the sake of having something to get mad about. Whether Napstablook recognized either of their cousins after they had become corporeal is not clear, but their tears help fend off the Mad Dummy when it tried to attack the human child. Later, during Mettaton's show, it is revealed that Napstablook is a fan of their cousin and called him to tell him how happy his show had made them, sad that Mettaton wanted to cancel it to become a celebrity on the surface. Along with other viewers sharing their gratitude for Mettaton, it caused him to reconsider moving up in his career.

In the console versions of the game, it is revealed that the Mad Ghost had left the dummy behind and exchanged it for a far more feminine body. It is unknown if Napstablook had heard of it, but the body is only revealed after Mettaton's second form is defeated, along with Mettaton interacting with Mad Mew Mew in the exclusive Alarm Clock dialogue outright confirming that he is aware of his cousin's new form.

Deltarune

In the world of Deltarune, the ghosts known as "Mettaton" and "Mad Mew Mew" in the Undertale timeline never got the chance to become corporeal, so just like Napstablook, they became anti-social shut-ins. It is revealed that Napstablook got a job on the Hometown police force, but talking to them reveals that they want to quit but can't since their cousins rely on them.

Fanon

Even prior to the reveal of Mad Mew Mew, the Ghost Cousins were a popular subject in fanart, particularly the idea of each ghost being color-coded, with Mettaton/Happstablook being pink, Napstablook being blue/white, the Mad Ghost being orange and the Ruins Dummy ghost being green. Some like to think what Napstablook would look like corporeal, but others prefer them to stay the way they are, so the three could form a trio of one male cousin, one female cousin and one neutral cousin.
