Smad Cousins is the familyship between Napstablook and Mad Mew Mew from the Undertale fandom.

Canon

Undertale

If the human had spared Napstablook during their encounter with them, they will return in the battle against the Mad Dummy. Stalling the Dummy for multiple turns will cause droplets to fall from the sky, which the Dummy mistakes for acid rain. After the Dummy had fled the scene, Napstablook will come down. Napstablook will assume that the Mad Dummy was just playing with the human and will feel bad about interrupting the "fun" they had.

If the player purchases the Mystery Key and finds Mettaton's old diaries, reading them will reveal that Mettaton and the ghost inside the Mad Dummy are Napstablook's cousins and used to live with Napstablook on the snail farm. However, one day, the Mad Ghost chose a training dummy as their corporeal form, leaving the farm. Napstablook was quite worried that Mettaton would want to become corporeal as well, as doing so would mean that Napstablook would be all alone.

Though Napstablook would have no way of knowing about this, the Mad Ghost chose another form in later console versions of the game. What ties the reborn "Mad Mew Mew" back to Napstablook is that they both prefer to be alone, yet being befriended by the human causes their lives to improve.

Alarm Clock App

In Mettaton's exclusive dialogue, he recounts a story of seeing Mad Mew Mew with Napstablook at Papyrus' Christmas party, throwing a fit since Mad Mew Mew supposedly wasn't invited. What really set Mettaton off was Mew Mew serving Napstablook some "ghost juice", only for it to phase through them and leave a mess on the floor. When the two started fighting, Napstablook fled the scene since they couldn't stand seeing their cousins fight. By the time the two had calmed down, Napstablook had already left and they go to the window to check on them. Staring at them, Napstablook's tears turn into ice crystals in the snow. Though Napstablook's exclusive dialogue clarifies that seeing the tears create something beautiful cheered them up, Mettaton and Mad Mew Mew nonetheless felt incredibly guilty for making a scene and causing Napstablook to flee the party.

Deltarune

In the Deltarune timeline, neither Mettaton nor Mew Mew ever became corporeal, thus they all live together in Napstablook's house as shut-ins. Napstablook got a job at the police station by complete accident, though they admit to hating it, they can't quit since they're their family's sole breadwinner.

Fanon

Very few pieces of fanwork ever feature Napstablook and Mew Mew interacting by themselves, they are often depicted as a trio of cousins with Mettaton. This can be explained by the two of them being minor characters, with their ties to Mettaton driving most of the headcanons involving them.

What can be said about their relationship in fanon is that it is often interpreted as healthy, as Mew Mew is never shown getting mad at Napstablook.
